The Daily Individual Reading Record is found in Ken Alford's Old Testament  - 
Seminary Scripture Mastery Resources! Volume One.  The Old Testament  Reading 
Chart Ark sheet, which comes full size (8.5 x 11 inch) and also 2 per  page 
for home use is found in Ken Alford's Old Testament - Seminary Scripture  
Mastery Resources! Volume Two.
 
I have found very useful and helpful ideas in all of his volumes for each  of 
the four years.  He has done a lot of time consuming and hard work to  put 
these volumes together so that we as teachers can use them and save  time.
 
This was money well spent on these 8 volumes.
 
Hope this helps you or anyone else looking for some ideas and help.
 
Also, one year as a reward for reading, I gave out RED treats.  For every 14 
days they Read Every Day, I gave them a treat that was red.  Gum, suckers,  
Lifesavers, taffy, Jolly Ranchers, etc.  I try to focus on reading as the  true 
incentive as they learn more for themselves than just doing it to get a  
treat, so that the treat doesn't become the main purpose.
