2 followed by the extension 3500. Then choose option 6, then option 2, or
Local Salt Lake City: 801-240-3500, option 6 then option 2

Because of privacy concerns and in order to locate your records you will be
asked to verify some of your personal information. Some of the information
you may need to have available is:

Full name of the priesthood holder
Date of birth
Current ward and stake
Current priesthood office held
Address and ZIP Code
Who performed the ordination

They will fax or mail the request to you.
